Does this sound crazy? That is exactly what I had to do today. A while back we were talking about students who were not really handicapped with mutism but rather selective.
I have a student who doesn't speak at school but she can speak outside of school. While she was my student in ES, I could normally read her lips because we were mostly dealing with individual words, but now she has reached JHS and students are speaking in sentences. Evidentally, the JHS had a meeting about it and decided that since she was not handicapped, she should be in regular classes. However, ALT classes are all about speaking and they just had their first speaking test. Honestly, how am I supposed to grade a student that mouths sentences to me? I'm supposed to be checking for pronunciation and grammar accuracy but for all I know she could be cussing at me in Japanese.

So just fail her. Don't fret over it, its not like they care about test scores anyway. But even if they do, it might be good to introduce her to the notion of consequences for her actions, or in this case inactions. If she really cares about her grades and everything, and the JTE's at your school take your speaking test scores as actual grades (I don't know how this works-my company actually forbids us for taking any part in the testing process) then maybe your whatever the japanese equivalent of an F is might just be the stimulus needed to get her to start speaking in school. Or maybe not...but it still might be make for small revenges.
